The dental van will be at John W. Cook Elementary School on the following dates

November 12th – 10 a.m. – 2 p.m.
November 13th – 3 p.m.  – 7 p.m.
November 20th – 3 p.m.  – 7 p.m.
December 10th – 10 a.m. – 2 p.m.
December 11th – 3 p.m.  – 7 p.m.

The van provides school dental exams & routine exams/cleanings.  A completed consent form, with a copy of medicaid/insurance card must be completed and return to the main office prior to each visit.  Consent forms are available in the main office.  Contact Mr. Wolfe in Room 112 (312) 513-7074.
