SCHOLARSHIP INFORMATION PACKET 2009-2010

Education for Excellence Scholarship Foundation, Inc. (the “Foundation”) will provide two (2) monetary awards to Chicago Public Schools High School seniors (excluding Chartered schools) who meet the requirements of the Foundation’s application process. The purpose of the monetary award is to assist the student with the expense and financial obligations of college attendance. The scholarships will be publicized through the Internet and print media.

ELIGIBILITY REQUIREMENTS: The Applicant must:

1. Be a high school senior attending a Chicago Public High School; Possess a cumulative Grade Point Average (G.P.A.) of not less than 2.5 on a 4.0 scale.

2. Complete and submit the following application packet information:

a. Submit two (2) type-written essays, (2 pages each, 12 font, Times New Roman style, double spaced, name in upper right hand corner.) First essay should include the Applicant’s Personal Statement that explains the Applicant’s financial need, current extracurricular activities, academics, future aspirations in post secondary education, and career goals. Second essay: What current event has interested you the most and why?

b. Submit two (2) letters of recommendation: One letter must come from a teacher or counselor who can account for the Applicant’s academics and who has known the applicant one year or more. Second letter must come from an adult who has known the Applicant for at least two (2) years and who is not a family member, teacher or principal. This person should be able to comment on the Applicant’s personal growth and achievements.

c. Provide a letter of intent to the college in which the Applicant wants to attend.

d. An Official 2009-2010 Transcript must be submitted in a sealed school envelope from your high school.

3. Applications are available between January 1 and April 1 of each year. All documents must be submitted together and received by May 15, 2010. Recipients will be notified by June 1, 2010.

4. All information submitted to the Foundation will be kept confidential and will not be returned to the applicant. The Foundation will mail or email an acknowledgement of the receipt of your application to the address provided. Incomplete or late applications will be disregarded. No telephone calls will be accepted.

5. Place your first and last name on all documents submitted as part of the application (i.e. all pages of the application, your essay question and letters of recommendation.)
